Create a form with date of birth

To do this:
Register with QForm and add a site where the form will be
Create a form and give it a name that is understandable to you. It is visible only to you
From the list of fields on the left, select "Drop-down list". Drag it to the designer field with the mouse or by clicking on the plus sign on the line with the field
If necessary, specify a date range. This will help avoid errors when users enter too high or too low an age.
Click "Save" in the upper right corner of the designer. Go to the "Design" tab and click on the "Date" field. Choose the calendar format to make it easy to fill out
Go to the "Insert Code" tab. Copy the text from the DIV window to install the form and the script to call the form, paste it into the desired place on the site
After pasting the code, the finished form will appear on the page.
